MEDICAL FORM-FILLING PROCESS:

Process type : Off-line Form-Filling

Workload : 200 Forms/system/Day.

Work Load per system : 200 forms daily

Minimum no of seats required : 5-10 agents + 1 server

Total workload : approx 38,000 forms for 10 machine

TAT : 22 days including Sunday and holidays

Rate per form : 50/- per form

Monthly billing on 10 machines : 22 Lack approx

Hardware requirements : Minimal, Internet is required only on Server

Company will issue demo work and provides training to the second party. After successfully completion of trail work company will issue commercial work to the second party thru FTP/DVD.

Accuracy: 100% to 99% - 50 Rs.

99.1% to 98% - 80% Payout.

98.1% to 95% - 60% Payout.

95.1%to 90% - 40% Payout.

Billing: - MONTHLY.

Setup Requirements: 10 Machines 512 MB RAM, Any PIV Machine with Windows XP or above
