Number of Forms Per System Per month: 550
Security Deposit with Bank Guarantee (Refundable) : Rs1000/ Per system
Number of entries per form: 12 to 33
Total Contract Period : 11 months
Turn around Time : 20 days
Required accuracy 100%
Payment terms : 15 days from invoice date.
Mode of Payment : Letter of credi ( Inland L.C)
Rate per form : INR 9/
Total billing per system : INR 4950

Nature of work : Hard copy of the united States Insurance forms to be filled in access database Through our customized software.

Duration of Agreement: 11months from the Effective date. Renewal with mutual agreement/consent

Date of commencement: Trial run commences on 25th June , commercial from 1st Of
July

Volume of work : 550 forms/seat/month

Quality, accuracy required. : 100% quality, Accuracy level of the work will be measured on the basis of individual forms.

Total monthly billing for 50 seats. 2,47,500


WORK DETAILS WITH SOFTWARE.

The Physical form is having 33 Fields, but for any patient record, all fields do not apply. Also there are fields restricted to the use of Insurance office, which need not be filled.

Out of the 33 fields, maximum of 18 fields ought to be typed, and the rest of the fields ought to be attended only by clicking selection buttons such as “yes” or “no”

SOFTWARE PROVIDED

Company will provide the software to enter data from the physical forms which will be stored In the database. Additional Administrator tools shall be provided to merge the data entered in the individual work stations at the end of each working day. The same has to be sent back to the company as per the delivery schedule.

Sign up will be in Madurai.
My consultancy charges are:

INR 10,000 to initiate the process and INR 50,000 At the time of sign up, for a slot of 50 seats and all paid expenses to Madurai and back . ( A/C train and Good Hotel accommodation)
